Health Examination Requirements

Physical exams are required for all students entering kindergarten, 6th and 9th grades. Students transferring from out of state or out of the country and students entering school for the first time regardless of grade must also meet this requirement. The exam must be dated within one year prior to the first student attendance date into these grades.

The IDPH Certificate of Child Health Examination form is the ONLY form that will be accepted. A sports physical form cannot be used to meet this requirement. However, the IDPH physical form can be used for sports physicals.

The physical exam form must be turned in by October 15.  If the physical exam form is not turned in by October 15, your child will NOT be allowed to return to school until the completed IDPH school examination form and complete immunization records have been turned in.

Dental exams are required for all students entering kindergarten, 2nd and 6th grades. The exam must be dated on or after November 15th of the year prior to entering these grades. 

Vision exams are required for all students entering kindergarten and students enrolling in an Illinois school for the first time regardless of grade. The exam must be dated within one year prior to the first student attendance date into school.

Immunization Requirements

  • DTaP (Diphtheria, Tetanus, Pertussis)
    Four or more doses with the last dose received on or after the 4th birthday

  • IPV/OPV (Polio)
    Three or more doses of all IPV or OPV with the last dose received on or after the 4th birthday
          OR
    Four or more doses of any combination of IPV and OPV with the last dose received on or after the 4th birthday

  • MMR (Measles, Mumps, Rubella)
    Two doses with the first dose on or after 1stbirthday and the second dose no less than 28 days after the first dose

  • Varicella
    Two doses with the first dose on or after 1st birthday and the second dose no less than 28 days after the first dose.  A history of chicken pox is acceptable if documented by your child's healthcare provider.

  • Hepatitis B
    Three doses required for entrance into 6th grade

  • Tdap (Tetanus, Diphtheria, Pertussis)
    One dose required for entrance into 6th grade

  • MCV4 (Meningococcal Congjugate Vaccine)
    One dose required for entrance into 6th grade; given on or after 11th birthday
    Two doses required for entrance into 12th grade.
